Referral from your Primary Care Provider

Income must be equal to or less than 200% of the Federal Poverty Level

Residency in Bay, Calhoun, Gulf, Franklin, Holmes, Jackson, Liberty, Wakulla, Walton, or Washington County

Must NOT have medical insurance of any kind
At BayCares, we want to bring more people together to help out and offer a wider range of specialty services for our patients. By increasing the number of providers who volunteer, we can make sure our patients have access to a variety of care options. Our goal is to build a community where everyone can find the help they need.
Qualification Process
After your Primary Care Provider faxes your referral to BayCares, a Care Coordinator will contact you to schedule your qualification appointment. Please plan to bring the following items to your first visit:

Social Security Card
& Photo ID
02.
Proof of Household Income (pay stubs, Social Security, child support, etc.)
03.
Food Stamp Letter
(if applicable)
04.
Proof of Medicaid Application
05.
Vocational Rehabilitation Letter
(if applicable)
